Peri-implantitis presents a growing challenge in implant dentistry, forcing clinicians to make complex decisions between preserving or removing dental implants. This lecture explores the biological, clinical, and patient-related factors that influence these critical choices. Attendees will gain insight into the etiology and progression of peri-implantitis and how early diagnosis impacts treatment outcomes. Evidence-based strategies for non-surgical and surgical management aimed at implant preservation will be discussed. The session will clarify when regenerative or resective therapies can offer predictable results. Equally important, clear criteria for determining when an implant has a poor prognosis and should be removed will be presented. Risk factors such as implant design, surface characteristics, systemic health, and maintenance compliance will be critically reviewed. Real-world clinical cases will illustrate decision-making pathways and long-term outcomes. The lecture emphasizes balancing biological feasibility with patient expectations and quality of life. Participants will leave with practical guidelines to support confident, patient-centered decisions in daily practice.
